Normal sudoku rules apply.
In each row and column, when a 9 is in the middle of a set of 3 adjacent digits the other 2 digits must sum to 9.
Killer cages: Digits inside a cage must sum to 9.
Sandwich sums: Digits outside the grid are equal to the sum of the digits appearing between 1 and 9 in the corresponding row or column.

Great puzzle. I really enjoyed the implications of the "tummy" restriction.
